Medication History Assist Software for Pharmacists Enhances Communications with Patients and Streamlines Medication History Preparation

TOKYO, September 20, 2018 ― PHC Corporation (hereafter PHC), a wholly-owned subsidiary of PHC Holdings Corporation (headquarters: Tokyo, Japan) announces “Medication History Assist,” new software that assists pharmacists in providing patient medication instructions. This product for the Japanese domestic market will be released on September 28.

“Medication History Assist” is software *1 that aims to enhance pharmacists’ medication instructions for patients. It is designed to increase patient understanding of medications as well as medication adherence by instructing via a tablet *2 correct dosages to patients by showing illustrated medication information. In addition, medication instructions can be copied to a medication history record on site automatically, reducing the time required to prepare medication history. Linking with PHC’s “Pharnes V-MX,” an electronic medication history system, pharmacists can conduct a full-fledged audit and prepare a more detailed medication history, depending on patient condition and pharmacy operation.

The latest medical fee system requires streamlining pharmacist tasks to further promote face-to-face communication with patients including home care visits and medication adherence instructions. “Medication History Assist” aims to provide more time for pharmacists to communicate with patients and offer better service by streamlining medication history preparation that can require over an hour a day for many pharmacists.
